To companion the end-of-life journey with skill and compassion.
Central Wyoming Hospice and Transitions Program (CWHTP) is a 501(c)3 nonprofit organization and is the premier provider of end of life care for individuals, their families and caregivers. With compassion, and collaboration, our team of professionals and volunteers offers a complete continuum of specialized care. Since 1981, CWHTP has been providing services to approximately 300 families in Central Wyoming annually. No patient is ever denied services because of the inability to pay.

Management Team
Marilyn Connor, CHPCA, Executive Director - marilync@cwhp.org
Marilyn has served as the Executive Director of Central Wyoming Hospice and Transitions Program since 2003. She has over twenty years of experience in nonprofit management and nursing. Marilyn, a Certified Hospice and Palliative Care Administrator, holds a BA in Organizational Management from Concordia University in Seward, Nebraska.
Deanne Haymond, Director of Administrative Services - deanneh@cwhp.org
Deanne has 25 years experience working with non-profits and has been with CWHTP since 2001. She has detailed experience in many areas of administration, including billing, volunteer coordination, bookkeeping and human resource management.
Tera Gillett, CSW, Director of Clinical Services - terag@cwhp.org
Tera has a B.S.in Psychology and a Bachelor of Social Work degree from UW and is a Certified Social Worker. Tera started at CWHTP as a Social Worker in 1993.
Jan Proper, MSN, RN, Director of Nursing - janp@cwhp.org
Jan is a graduate of Casper College where she earned an associate's degree in nursing. Since that time Jan has attained a master's in social work and in nursing. Jan has spent many years working in critical care and worked as a nursing instructor at Casper College.
