What is Outpatient Hospice?
Outpatient hospice care is care that is performed within your place of residence, whether it's a home, apartment, nursing home, or assisted living facility.
Our dedicated team of nurses, social workers, volunteers and staff will visit you to provide medical care, assistance, companionship, spiritual services, counselling, and support. In addition, nurses are available 24 hours a day, seven days a week to deal with emergencies.
Outpatient hospice care allows the patient to stay in their own home, surrounded by the people they love, while still receiving the care and services they need.

Central Wyoming Hospice & Transitions services are administered by our dedicated staff of nurses, social workers, Hospice aides, chaplains, counselors, and volunteers. We provide:

Is Hospice Right for Me?
If you're not sure if Hospice is right for you, see if these statements are true:
- Diagnosed with a terminal illness, such as heart, lung, kidney and liver disease; all types of cancer; Alzheimer's; or dementia.
- No longer seeking curative treatment.
Do I Qualify?
Anyone diagnosed with a terminal illness, a life expectancy of six months or less, and who is no longer seeking curative treatment qualifies for hospice care.
Who Pays for Outpatient Hospice Care?
MEDICARE & MEDICAID: Patients with Medicare or Medicaid are typically covered without patient co-payments. Medicare and Medicaid cover outpatient clinical and nursing services, drugs related to the terminal illness, and equipment.
PRIVATE INSURANCE: Many private insurance policies include coverage for hospice outpatient care.
Central Wyoming Hospice & Transitions is a non-profit organization. Through the gracious financial support of our community, we never turn away an eligible patient because of their inability to pay.
Advanced Directive
An advanced directive is a document that instructs your doctor and your family of your wishes concerning end of life decisions. An advanced directive may also appoint someone to make your health care decisions for you if you are unable to make them yourself. An advanced directives form is available on our website here.
